Overview

Therapeutic targeting is the design and deployment of interventions that act selectively on a defined molecular or cellular feature driving disease, with the aim of correcting a pathological process while sparing healthy tissue. The approach begins with identifying and validating a target, typically a protein, signaling pathway, metabolic dependency, gene product, or cell population whose modulation alters the disease course. Once validated, agents such as small molecules, monoclonal antibodies, peptides, or nucleic-acid therapeutics are developed to inhibit, activate, or otherwise perturb that target. Common strategies include blocking enzymes and receptors, exploiting metabolic vulnerabilities on which malignant or stressed cells depend, modulating transcription factors and inflammatory regulators such as NF-κB signaling, and interrupting host pathways co-opted by oncogenic viruses. Rational target selection is increasingly informed by structure-based and computational design integrated with in silico, in vitro, and in vivo evaluation, allowing candidate compounds to be screened, optimized for potency and selectivity, and assessed for efficacy and safety. Therapeutic targeting underpins precision and personalized medicine, in which treatment is matched to the molecular profile of a patient or tumor. Its central challenges include achieving specificity, overcoming acquired resistance, managing off-target effects, and translating mechanistic insight into durable clinical benefit across oncology, immunology, and infectious disease.
