Overview
Cerebrovascular disease comprises disorders of the blood vessels supplying the brain that compromise cerebral perfusion, producing ischemic or hemorrhagic injury to neural tissue. It is a principal cause of death and acquired disability worldwide and encompasses ischemic stroke and its subtypes, intracerebral and subarachnoid hemorrhage, transient ischemic attack, and chronic small-vessel disease that contributes to vascular cognitive impairment. Pathogenesis involves atherosclerosis, thromboembolism, endothelial dysfunction, and hypertensive and metabolic vascular damage, with risk modulated by hypertension, diabetes, hyperhomocysteinemia, dyslipidemia, and genetic factors such as polymorphisms in the angiotensin-converting enzyme gene. Diagnostic and mechanistic research examines endothelial function across stroke subtypes, carotid intima-media thickness, and the consequences of vascular insult to specific brainstem and oculomotor pathways, as in infarction presenting with internuclear ophthalmoplegia. Cerebrovascular injury also intersects with neurodegenerative and inflammatory disease and with systemic conditions including chronic kidney disease and acute infection. Antioxidant and radical-scavenging strategies are investigated for their potential to limit ischemic damage. The journal publishes peer-reviewed clinical, mechanistic, and case-based studies addressing stroke and its subtypes, vascular risk factors, endothelial and hemodynamic assessment, and the neurological consequences of disrupted cerebral blood supply.
