Overview

Cystectomy is the surgical removal of the urinary bladder, performed as either a partial cystectomy, in which a localized portion of the bladder wall is excised, or a radical cystectomy, in which the entire bladder is removed, typically together with adjacent structures such as the prostate and seminal vesicles in men or the uterus and a portion of the vagina in women, along with regional pelvic lymph nodes. The most common indication is muscle-invasive or refractory bladder cancer, while other indications include selected cases of severe radiation or chemical cystitis, intractable bleeding, and certain congenital or functional bladder disorders. Because the reservoir for urine is removed, radical cystectomy requires concurrent urinary diversion to reroute urine from the ureters. Diversion options include an incontinent ileal conduit that drains to an external stoma, a continent cutaneous reservoir, and an orthotopic neobladder constructed from bowel and anastomosed to the urethra to permit more natural voiding; each carries distinct functional outcomes and long-term considerations, including metabolic effects and the potential for stones within bowel-derived reservoirs. The procedure may be performed open, laparoscopically, or with robotic assistance. Perioperative management addresses bleeding, infection, bowel and urinary anastomotic integrity, and rehabilitation, with oncologic outcomes depending on tumor stage and the completeness of resection.
