Overview

Patient safety is the discipline of preventing, reducing, and managing errors and avoidable harm in healthcare. It addresses risks such as medication errors, adverse drug events, healthcare-associated infections, and breakdowns in communication, and it relies on practices including risk assessment, clear documentation and standardization, safe medication administration, and systems designed for high reliability.
