Overview
Patient safety is the discipline of preventing, reducing, and managing errors and avoidable harm in healthcare. It addresses risks such as medication errors, adverse drug events, healthcare-associated infections, and breakdowns in communication, and it relies on practices including risk assessment, clear documentation and standardization, safe medication administration, and systems designed for high reliability. The goal is to protect patients from unintended harm while care is delivered, recognizing that safe systems and processes are as important as individual clinical decisions. Research within Patient Care and Services covers many facets of patient safety. Reported work includes patient safety and clinical risk management in general practice, evidence-based commentary on high reliability in healthcare, the standardization and codification of adverse drug event definitions and documentation, evaluation of quality improvement strategies in healthcare facilities, and antimicrobial stewardship practices among prescribers.
