Overview

Bipolar disorder is a chronic, recurrent mood disorder defined by alternating episodes of elevated mood, manic or hypomanic, and episodes of depression, interspersed with periods of relative stability. The diagnostic spectrum distinguishes bipolar I disorder, marked by full manic episodes, from bipolar II disorder, characterized by hypomania with major depression, and from cyclothymia, a milder but persistent pattern of mood instability. Its etiology is multifactorial, reflecting a substantial genetic contribution alongside neurobiological dysregulation of neurotransmitter, circadian, and neuroendocrine systems. Pharmacotherapy with mood stabilizers, often supplemented by antipsychotic or antidepressant adjuncts, forms the foundation of treatment, but durable outcomes depend on combining medication with structured psychosocial interventions. Such interventions, including psychoeducation and approaches that address relationships, routines, and early warning signs, help patients manage stressors, sustain adherence, and reduce the likelihood of relapse. Functional recovery is shaped not only by symptom control but by psychosocial factors and the frequent presence of comorbid psychiatric and medical conditions, which complicate the course and influence quality of life.
